[fitswcs] Slant zenithal projections

I've generalized AZP to tilted planes of projection and also introduced
SZP.

Text changed since the 2000/11/07 draft is colour-coded blue.  This is
done manually and does not include some trivial changes.  Text which has
been removed is marked by surrounding words or phrases so you need a copy
of the older draft for a proper comparison.
