[daip] lost file names using FITAB and FITLD

A question before I start digging - do you write the tape in one go with 
FITAB or multiple executions?  And do you read it in one go or multiple 
executions of FITLD?

And is it always this pattern - where the first name is recovered and 
the later ones lost?
