[fitswcs] Paper III

I will shortly be away from e-mail for a period of time (late 19
through all 27 Sept) and then away from home until mid October.  The
referee has not responded, but considerations raised by users of -TAB
have led to a small change that allows for small extrapolation of the
-TAB coordinates (previously we forbad that).  The new version is
