Sigh...

I found 2 places in the code that messed up the dimensions in the
header when the number of pixels in the Dec was less than that in
RA.  My test case had the 2 equal and so this "typo" was not found.
I did have another test case where NY < NX by a good bit and that
failed far less gently than your case.  It should be fixed  tomorrow.
