New Staff Reply: DTSIM bug and explain information errors

I have completed an overhaul of the "standard" antenna locations
which was why u,v,w seemed wrong.  (The VLA coordinates were quite
wrong.)  I have improved the explain file significantly and added a
real example - the one I used in debugging.  A MNJ on 31DEC12
tomorrow will get the updated version.

Thanks for prompting me to work on this task - it is a better task
than I had realized.
