Its a pity that the writer wasn't required to actually use the
data-sheet to design something...  Anyway, I agree with your
assessment, so the update to my external hardware interfaces document,
which I just released, shows the AD8138 being supplied for a single
+5V supply. I also agree with your point about the clamp diodes, and
have included those to my circuit.
