[fitswcs] Adaptive mesh refinement

I should have paid more attention to what I wrote before hitting the "Send" 
button.  The last thing I wrote was

I think this is a mistake.  I think the correct dimensionality would be 
(3,1000,1,1).  However, the documentation disallows degenerate axes, so we'd 
have to substitute (3,1000,2,2), of which only 25% is actually used.

Hmmm, maybe pixel lists would make more sense.
