[fitswcs] -TAB progress report

I have made some changes to the text:

1. To correct the header in Table 11 and to add a sentence pointing
out the use of a degenerate axis whose values vary with a different
pixel axis.

2. A new paragraph to mention the inverse problem in -TAB
implementation

3. To forbid repeated index values when M > 1.

4. To forbid Km = 1 in multi-axis dependent lookups.
