[fitswcs] revised ccs sect. 2

[Mark Calabretta]

I've divided Section 2 into subsections to make it a bit easier to see the
logical structure, rewritten section 2.2 to explain the whys and wherefores
of the reference point and to introduce user-specified reference points,
generalized 2.4 to suit, and added 2.7.
