One side note: I cannot see how DS9 could have generated these headers. I’ve checked the source code up to 6 years back, and I see no code that would have added or altered a NAXIS3 header keyword with value 0.

The only weirdness I can generate is when a user uses the 3D Axis Reorder function on a 2D image. Next release will disable this function to prevent clueless users from doing stupid things.
