There is a task called REMAP in my WCSTools package which does exactly
what you want to do. The package is described at

with a brief description of remap at

REMAP supports the full set of projections in paper II plus the
DSS plate coefficients and the IRAF TNX polynomial-corrected projection
for input and the paper II projections for output. This is not the
most documented or debugged task in the package, but the subroutines
on which it is based are pretty widely used.
