The ftp3d routine has been widely used by other users.  An example of how to
write a 2-D image with FITSIO is given in the Cookbook that is available from 
http://legacy.gsfc.nasa.gov/docs/software/fitsio/fitsio.html; this example
could be easily extended to the 3-D case.  If you still have problems, send me
more details of what you are doing and I'll try to help.
