[evlatests] New L352 Configuration

The two newest L352s (SN005, MIB 1-70 and SN006, MIB 1-71) have been 
installed, replacing two of the older units (SN001, MIB 0-3D and SN004, 
MIB 0-52).  The current configuration is:

ea13-l352 (SN003, MIB 0-51) is monitoring Ant13
ea14-l352 (SN002, MIB 0-40) is monitoring Ant14

ea16-l352 (SN005, MIB 1-70) and
ea18-l352 (SN006, MIB 1-71) are monitoring the two fibers from Ant16

All four are available from SlotID.

Two Round Trip Phase monitor points are available from the archive: 
PHASE_SAMPLE, which is archived once per second, and
PHASE_AVERAGE, which is a 10-point moving average, and is archived once 
per 10 seconds.
