[evla-sw-discuss] Address/hostname table for EVLA Widar0

The failed board in rack 002, slot 0 now works and gets
both serial and rack/crate/slot based addresses.

There still seems to be an issue with them getting their
rack/crate/slot address on boot.  Script has to be run
manually.
