Thank you, Eric and Amy, you have been most helpful. The different phase pointings in this dataset are different correlator passes on the same set of observations, rather than being different physical pointings of the telescopes. The times of the scans on all pointings {E4,E3,…,C0,…,W3,W4} are the same, so I can fix the problem by using TABED to alter the Source ID.
However, for some of the epochs (BB301F, BB301G, BB301H and BB301I), phase pointings SS433_E4 and SS433_E3 have the same Source ID. For example, here are the appropriate lines from the output of LISTR for BB301I:

This is rather bizarre and perplexing, and only affects some of the epochs (which were otherwise loaded through an identical pipeline). Is it something that you have seen before?
