[daip] LWPLA rpoduces output file when it shouldn't

I have changed the task to defer opening the output until the first
file is opened and the first record read.  Actually the empty file
written previously caused no problems - a corrected PLVER would append
correctly to the 0-byte file.  But it is confusing.
